The Limitations of the Sex Addiction Model

The sex addiction model used to be the only perspective when treating out of control sexual behaviors.

While some therapists may find the model useful, it has limitations. It focuses on pathology and doesn’t offer a definition of healthy sexual behavior or a specific pathway to guide clients toward achieving it.

The sex addiction model also lacks research to support it, and this has led to the term not being endorsed as a clinical diagnosis by The American Psychiatric Association in the DSM or the classification system (ICD 11) developed by the World Health Organization.

Despite its limitations, the model and the term “sex addiction” is still used extensively by professionals and this is unlikely to change any time soon.

However, there is an alternative…

THE OUT OF CONTROL SEXUAL BEHAVIOR (OCSB)MODEL

The Out of Control Sexual Behavior (OCSB) model expands upon the sex addiction perspective.  

It focuses on a problem level, not a disordered level. It’s an evidence-based model that is non-pathologizing, non-shaming, and sex-positive. It focuses on sexual health instead of the absence of a disorder.

The model has 6 principles to guide men in managing their out of control sexual behaviors while still focusing on sexual pleasure.

*NOTE: the model is NOT intended to be used for non-consensual sexual behaviors, including the spectrum of predatory sexual crimes of pedophilia, violence, and rape.

Before taking Doug’s training, I worked in a sex addiction program, and the model wasn’t effective with my clients. Individuals would repeatedly return with the same problem sexual issues, burdened with shame. Given my training as a sexologist, the sex addiction model also didn’t make sense to me, but I didn’t know of any alternatives. I concluded that I was a terrible therapist because my clients failed to gain control of their out-of-control sexual behaviors. 
 
Ultimately, I left my job at the sex addiction clinic, nearly abandoning my career as a therapist. Fortunately, a colleague later introduced me to Doug’s sexual health model for treating individuals with problem sexual behaviors, and I eventually took his training. 
 
I experienced immense relief as this model finally resonated with me, and I discovered that I was actually a pretty good therapist. Doug’s model was aligned with my knowledge of sexology and was grounded in evidence-based practices. 
 
I now listen to my clients as opposed to listening to the unscientific sex addiction theory. The clients I currently work with are more comfortable opening up in therapy, liberated from the overwhelming shame most have carried for so long. Most importantly, they regain control over their previously unmanageable sexual behaviors.

The OCSB model is primarily used with heterosexual or gay men as they are the population that presents with OCSB most often. However, the model can be used with any population. If you work with men’s partners or couples, you’ll definitely find it useful.

If you work with men individually or in couples therapy, this course can be useful to you as it will provide a framework to assess and treat men who have problematic sexual behaviors. You do not need to specialize in treating men and you do not need to become an expert in the model to use it with your clients.

The course can be helpful when working with men’s partners individually, helping them understand and support their partner in achieving healthy sexual behaviors.

The course will also be useful if you haven’t studied any human sexuality research and how it relates to out of control sexual behaviors.

Here are the most common out of control sexual behaviors that the OCSB model can help men with. NOTE: the behaviors below may not be problematic in themselves, it’s only when they become out of control or men feel they are out of control that they become a problem.

  • Use of sexual imagery to enhance solo-sex
  • One-time sex with a new partner

  • Hidden erotic and sexual interests
